If you are unsure who your plan administrator is, refer to your Collective Bargaining Agreement, speak to your steward or Union representative.
Please check your Collective Bargaining Agreement to determine if you are eligible for benefits (e.g., passed probation, work enough hours, etc.).

If you need to submit your enrolment form directly to Global Benefits you can send it Mail, Fax, or Email to benefits@globalben.com.
NOTE: Dental Providers should submit electronically through the EDI System.
Submitting Claims
The drug card is used for prescriptions at your local pharmacy, and it also is used by the dental office to communicate with Global Benefits.
For claims that need to be sent to Global Benefits (paramedical, etc.) you have several options including the Global Benefits Online Portal (via their website), the Mobile App, mail, and fax. You will need original receipts and service provider statements.
Global Benefits Online Portal
To submit claims online, you will need to have already been enrolled to register an online account, and have a drug card number or Member ID.
Global Benefits Mobile App
Global Benefits has created a video providing a guided tour of the mobile app.
The app allows you to create a new claim, view your Explanation of Benefits (EOB), look up providers, review your eligibility / coverage status and coverage expiry date, and more. It is important to note that the expiry date will change as you continue working (or stop working).
Most of these features are also available through the Global Benefits Web Portal.
Mailing or Faxing Claims
Mailing or faxing claims remains an option. It is important when sending in the claims/documents/receipts, that you provide your full name, current complete correct address and either your drug card number or S.I.N.
If you are submitting claims by mail, we recommend you retain copies of the receipts and the form(s).

Global Benefits prefer members not use email to submit claims to avoid exposing any sensitive or private information. If you have no other means to submit a claim, you can email the completed claim form, statements and receipts to seiubenefittrust@globalben.com.
Claim Forms
If you are submitting your claim via mail or fax (or email), you will need to fill out a claim form.
Please note Global Benefits no longer provides dental claim forms as it is a standard practice for all dental practitioners in Canada to submit claims to Global Benefits. If you need assistance, call Global Benefits.
Some members have Short Term Disability benefits in their Collective Bargaining Agreement. Check your Global Benefits Schedule of Benefits to verify your eligibility or contact Global Benefits for more information.

Justice for Janitors
Through collective bargaining in 2025, we negotiated improvements to the benefit coverage available to members and eligible dependents in many of our bargaining units.
In addition to some significant improvements to the schedule of benefits, the most exciting aspect of the improvements is that with the introduction of a new system, the Benefit Reserve Fund, hundreds of members and their dependents who were not previously eligible for benefits, will now be covered.
